The D4 Medical Examiner Form
You need to bring a D4 medical form along to your medical assessment, and they’re easy to get hold of. Drivers can download the DVLA D4 medical assessment report on the government website. Just print it out and bring it along to your D4 driver medial assessment. If you’re renewing your licence, the DVLA will usually send you a copy a few weeks before the licence renewal date.
What Do I Need to Take to an HGV Medical?
You will need your D4 medical assessment form, photo ID (your passport or photo driving licence), medical history notes, details of any medications you’re currently taking, plus your glasses and latest opticians’ prescription if you need them.
How Long Does the HGV and Bus Medical Take?

What Happens at a D4 Medical Assessment?
The doctor will review your medical notes and check your current health, including a blood pressure test. The DVLA requires that HGV and bus drivers have a blood pressure of 179/99 mmHg or lower. You will also have to pass the DVLA eye test by achieving 6/7.5 (6/60 is accepted for the eye with weaker eyesight).
When Will I Get My Completed D4 Medical Form?
